Recently, CareSource informed us that your family is in jeopardy of losing your YMCA membership because your Well Visit requirement has not been met. You may recall receiving a letter indicating that to continue your YMCA membership and other services, Well Visits need to be completed by September 1st. 
 

Membership Well Visits to Complete Within the Year
YMCA One Well Visit with your doctor 
AND
One dental visit with a dentist

We don't want to lose you as a Y member!
